Use the following link to submit payroll forms such as name changes, direct deposit, and tax forms (only if you are unable to sign-up via employee self service – UNCGenie.)

Please be sure to upload the following files and copies (if they are applicable to you):

• Name Change Form (must provide copy of Social Security Card)
• Address Change Form
• IRS W-2 Paper Request From
• Federal & State Tax Forms (only if unable to sign up via employee self-service – UNCGenie)

  • State Tax Form – NC-4EZ or NC-4-Web
